Intensive Outpatient Program (IOP)

An Intensive Outpatient Program (IOP) for substance abuse is a structured treatment program designed to provide comprehensive support and therapy to individuals struggling with addiction while allowing them to maintain their daily routines, such as work, school, or family responsibilities. Here’s a detailed description of what an IOP typically entails:

  1. Structured Treatment Sessions: Participants attend regularly scheduled sessions multiple times per week, typically in the evenings or on weekends, allowing them to continue with their daily commitments.
  2. Individualized Treatment Plans: Each participant receives an individualized treatment plan tailored to their specific needs, goals, and level of addiction. This plan may include a combination of group therapy, individual counseling, educational workshops, and specialized services.
  3. Group Therapy: Group therapy sessions are a central component of an IOP, providing participants with opportunities to connect with others facing similar challenges, share experiences, and offer support to one another under the guidance of trained therapists.
  4. Individual Counseling: Participants also receive one-on-one counseling sessions with a licensed therapist to address personal issues, develop coping strategies, and work through underlying psychological or emotional factors contributing to their substance abuse.

Targeted Case Management (TCM) services provide additional support that an individual may need to navigate complicated systems. A targeted case manager at Louisville Health & Healing can provide a multitude of services including assisting with acquire needed benefits, addressing vocational needs, addressing educational needs, housing assistance, assistance healthcare needs, and much more!

Targeted Case Management services are for adults who have Medicaid insurance along with a diagnosis of a severe mental illness or substance use disorder. TCM would meet with you to determine eligibility for services and develop a critical needs assessment along with a service plan to best address your needs. This service is very complimentary and enhances counseling and therapy sessions.

Ketamine Assisted Psychotherapy

The administration of ketamine in lower, sub-anesthetic doses to treat pain, depression, or other psychiatric diagnoses is a newer use of ketamine. Psychiatric use of ketamine has become relatively wide-spread in recent years, has been studied and promoted by researchers at the National institute of Mental Health, and has had front-page publicity as the newest anti-depressant. Ketamine has been administered by intravenous (IV), intramuscular (IM), sub-lingual, oral, and intra-nasal routes. Often, it has been used after other treatment approaches have been unsuccessful. Ketamine, approved by the FDA for treatment-resistant depression” is now an increasingly clinically applied “off-label” treatment for various chronic “treatment-resistant” mental conditions.

Ketamine is a Schedule III medication that has long been used safely as an anesthetic and analgesic agent and now, often effectively for treatment of depression, alcoholism, substance dependencies, PTSD and other psychiatric diagnoses. Ketamine-assisted psychotherapy is increasingly recognized as helpful in the emotional transition when end-of-life is in sight. Ketamine is classified as a dissociative anesthetic, dissociation meaning a sense of disconnection from one’s ordinary reality and usual self. At the dosage level administered to you, you will most likely experience mild anesthetic, anxiolytic, antidepressant and, potentially, psychedelic effects. Recent work has demonstrated of an anti-depressant and anti-suicide response to low dosages (that is, low dose compared to anesthetic usage) of ketamine– administered intravenously, intra-nasally and sublingually (orally)–that produce minimal psychedelic effects; this anti-depressant effect tending to be more sustained with repeated use—in other words, a cumulative effect.

It is our view that psychedelic, ‘dissociative’ experiences may well be instrumental in providing a more robust effect. This may well include a positive change in outlook and character that we term a ‘transformative’ response. We may employ both methods together as will be described herein. Essential to both methods is a time-out of usual experience, this period being of varying duration, usually 30 minutes to 2 hours, that tends to be dose and method of administration related. Relaxation from ordinary concerns and usual mind, while maintaining conscious awareness of the flow of mind under the influence of ketamine is characteristic. This tends to lead to a disruption of negative feelings and obsessional preoccupations. It is our view that this relief and the exploration and experience of other possible states of consciousness are singularly impactful.

EMDR Intensives

EMDR Intensives

We believe that one of the kindest, most efficient, and effective ways to overcome distressing emotions and decrease/ eliminate symptoms is through EMDR Intensive Therapy. EMDR (Eye Movement Desensitization and Reprocessing) therapy is an integrative psychotherapy approach that has been widely researched and proven effective for the treatment of trauma. EMDR Intensive therapy is designed to accelerate the healing process by offering several therapy sessions over a short period of time. Instead of the typical weekly therapy structure, clients dedicate 1-3 days for several hours per day to focus on a particular therapeutic issue.

Situations where you might choose an EMDR Intensive include: 

  • Recent upsetting events have disrupted your day-to-day functioning and you want to feel better and return to previous level of functioning faster .
  • You have witnessed or experienced a disturbing or traumatic event in your life and have symptoms that bother you (ie. car accident, work related events in first responders, sexual assault, crime, traumatic medical procedure, severe injury, violence, childhood bullying, military combat, emotional abuse).
  • A week-to-week therapy schedule doesn’t work for you.
  • You don’t have access to a trauma therapist but are able to spend dedicated time to travel and work in a more focused and succinct way.

Benefits to using an Intensive Therapy Approach: 

  • Reprocess more information in a shorter amount of time and feel better faster: Dedicate 1-3 days to focus on healing, growth, and connection.
  • Cost effective: Intensives are highly concentrated and focused, sometimes requiring less time and overall cost to the client. No more weekly appointments.
  • If full day appointments (6 hour sessions) are not doable, then we offer Extended sessions. Extended sessions are 2, 3 or 4 hour sessions.

Art Therapy

Art Therapy

Art therapy is a true gift to the service of healing and does not require any artistic skill whatsoever; art therapy focuses on the process, not the product. The quality of a client’s art is not the focus, as the experiences and insight gained through the exercise is what informs progress.

Art therapy has been proven to be an extremely effective modality for individuals of all ages, with varying presenting issues. Some issues that art therapy works to treat include, but are not limited to, those actively processing trauma, behavioral/emotional issues, anger/aggression, addiction, grief/loss, depression, anxiety, and ADHD. A major benefit to this expressive modality is that it will often elicit responses that have been challenging to obtain independently or through other therapeutic techniques. An art therapy session can look many different ways. Whether it be painting, wood-working, drawing, sculpting, or any other media used, the creative process offers new perspectives and catharsis that can be paramount to an individual’s treatment.

Moderation Management

Moderation Management

Problems with alcohol can take many forms. When problem drinking creates issues an individual either wants or needs to address, they have a few options to consider. The most well-known model of recovery from problems with alcohol is the 12 steps of Alcoholics Anonymous, whose end goal is total abstinence from alcohol. For some, that approach seems either too extreme or entirely unrealistic. 

Moderation Management (MM) takes a different approach. Rather than requiring complete abstinence, those practicing MM attempt to reduce their consumption of alcohol and problematic behaviors associated with their alcohol use with the support of a community.  It is an individualized program that allows problem drinkers to attempt a healthier relationship with alcohol.  By looking at one’s relationship with alcohol in a supportive environment, problem drinkers can determine for themselves what is working, what is not, and how to proceed in the future   to create a healthier relationship with alcohol. 

A therapist who understands the goals of MM can be an asset in achieving both short-term and long-term goals of those who seek to moderate their drinking. A compassionate and empathetic therapist who understands the values and mission of MM is imperative to building the supportive network necessary to address one’s issues with alcohol.

Psychedelic Support

Psychedelic substances, or medicines, have been used throughout human history by Indigenous peoples across the globe. They have been used to create spiritual and communal connections and heal psychological and physiological problems.  Western medicine has only recently embraced the therapeutic benefits of psychotropic compounds such as cannabis, psylocibin, ketamine, ayahuasca, peyote, and MDMA.  Research indicates these compounds provide many uses in treating both mental and physical health issues, something Indigenous communities have known for generations. However, these substances are still illegal in most of the United States.

Psychedelic support involves integration of psychedelic experiences and reducing harm from consuming these substances. There can be painful or difficult experiences which are hard to navigate once the experience is over. Having a therapist who understands the use of psychedelic substances is an invaluable resource. By utilizing them, you open yourself up to powerful psychological and spiritual experiences. A therapist who can support and encourage your path of personal development can create a safe and healing environment to process those experiences, while helping to mitigate any potential harm. 

Psychedelic-Assisted Therapy is an emerging modality that has been used to treat depression, addiction, anxiety and Post-Traumatic Stress Disorder (PTSD). There are several places offering Ketamine infusions to treat treatment resistant depression; individuals may have been part of clinical trials with the organization Multidisciplinary Association of Psychedelic Studies (MAPS); or have used these substances for recreational use. We do not offer Psychedelic-Assisted Therapy but we do offer integrated support for those that need help making sense of their experiences. These sessions can be a powerful and helpful way to unpack the clarity and awareness that happens through these experiences.

Recovery Integration

Recovery Integration

People who achieve long-term addiction recovery often benefit from individual therapy. Unfortunately, those in recovery can have difficulty accessing therapy unique to their needs. Recovery integration pairs a person in recovery with a therapist who is familiar with, and supportive of, different recovery programs.  Life problems do not end with the completion of inpatient or outpatient treatment programs. Recovery integration supports and guides each client though the ups and downs of daily living, applying principles of recovery and reformatting patterns that can lead to the despair of active addiction.

Finding a therapist who is fluent in the language of recovery can allow individuals to be more open and vulnerable while establishing a therapeutic relationship. Those in recovery can address their thoughts, feelings, and issues in a safe environment, customized to their individual needs and within a framework that supports their ongoing recovery. From reentering society from treatment to someone with years of recovery experience under their belt, recovery integration supports recovery as a general framework and guide for living.  Individuals can learn how to incorporate newly learned principles and skills into their home, work, and social lives with the support of a therapist who understands their goals. We have multiple therapist that are active in recovery and practice integration daily. We find that it is important to have licensed professionals that understand substance abuse and recovery both, professionally and personally. Identification is the antidote to loneliness.

Addiction

Addiction

Addiction is a complicated brain disease involving compulsive use of substances or harmful behavior patterns despite negative consequences. A person with addiction suffers from one or more of the following symptoms: social difficulties, impaired control, risky use, withdrawal, and increased tolerance. Continued use not only impacts one’s ability to be successful in society at large, but creates harmful, biological changes to the brain and its ability to function. These changes include impaired judgement and decision making, damaging behavioral issues, memory loss, and emotional problems. 

Due to the complex nature of addiction, recovery requires different types of treatment to address its mental, physical, and emotional components. The recovery process begins with detox and addressing the physical withdrawal of substances and behaviors. Once removed, it becomes imperative to treat the mental and emotional issues that arise. People with addiction often use substances and behaviors to both self-medicate, and to escape emotional pain, difficult situations, and traumatic events. Individual therapy allows clients to process through emotions and issues which have long been hidden by substances. Individuals can also identify and address triggers and cravings, help avoid relapses, and learn new coping mechanisms necessary for long term recovery. 

Addiction recovery can be a lifelong process, requiring a multitude of therapies and opportunities for growth. Addiction affects the sufferer as well as their friends and families. Individual therapy can also help restore relationships and heal wounds caused by the disease of addiction and its effects in our lives. Regardless of one’s substances of choice or behavioral addiction, recovery is possible and individual therapy can be an important part of long-term recovery.

The Addiction can have a negative meaning and people often report the stigma around talking about their substance use problem. We are compassionate and non-judgmental in out approach when treating person who struggle with substance abuse. People may also struggle with non-substance addictions like over-eating, excessive spending, gambling, internet, gaming – just to name a few. We also work with these issues as they have similar effects on the brain and biology of people.
